How to Read a Police Report in Washington State

Washington State police reports are critical documents that provide factual information about an incident or an accident. A knowledgeable Washington personal Injury Attorney will know how to read them by heart, however claimants will often not know what everything means especially the numbers or boxes in the report. Key Identifiers on a Police Report in Washington State: 

  • REPORT NUMBER - a pre-printed number assigned by the State of Washington and appears in the upper right corner of all pages.
  • DATE - is a mandatory field. 
  • TIME - is entered in a 24-hour military time format. (For example 1645 is 4:45 pm), If time is unknown for daytime the officer will enter 13:00 and nighttime  00:01. If no information regarding time is available at all it will remain blank.

A. LOCATION INFORMATION 

The officer will select the category of roadway such as:

  • State route
  • County road
  • City street
  • Private way or other.

B. COLLISION INCIDENT TYPES 

One or more of the bellow may be marked by the officer:

H. INTERSECTION

  • If the collision occurred within 20 feet of/or in an intersection, only the primary traffic way and cross-street will be filled out by the officer. 
  • If the collision occurred at a non-intersection location, the distance in miles or feet, and the direction (north, south, east, west), to the nearest cross street will be provided in the police report.  
  • If available, the officer will provide the block number or milepost.

J. PRIMARY TRAFFIC WAY

The officer will indicate the highway, road, or street on which the collision occurred on which is the primary traffic way. 

K. BLOCK NUMBER OR MILE POST

The officer will indicate if the collision occurred on a street with a block number or on an interstate with a mile post. An approximate mile post can be used by the officer as well.

L. DISTANCE AND DIRECTION

The officer will document the direction and distance to the nearest cross street or reference point to the collision site. The officer may check more than one direction such as north and west for northwest.

M. CROSS STREET

The officer will enter the name of the cross street or reference point.

UNIT INFORMATION

  • Damage Threshold - The officer will check yes, if the total damage to any one vehicle involved in the collision is estimated to exceed a cost of $700 or more
  • Commercial Driver’s License (CDL), Endorsements, and Restrictions will be recorded by the officer. 
  • Driver’s License and State - The officer will have this information recorded or will indicate if the operator does not possess a driver’s license [No Valid Operator’s License NVOL] or its expired at the time of the collision.
  •  On Duty - The officer will check this box if the driver was an on-duty such as:  law enforcement officer or fire fighter at the time of the collision. 

Airbags

The officer will write the numeric airbag code that best describes the vehicle’s airbag system. The airbag codes are as follows:

  • (1) Not Airbag Equipped
  • (2) Not Deployed
  • (3) Deployed – Front
  • (4) Deployed – Side
  • (5) Deployed – Other
  • (6) Deployed – Combination
  • (9) Deployed – Unknown

Restraint Systems

The officer will write the numeric restraint system code that best describes the restraint system. The restraint codes are as follows:

The officer will enter this info only if the unit is a motorcyclist, pedalcyclist, skater, or skateboarder with the following codes:

INJURY CLASS

The officer will enter the numeric code for the category that best describes the injury. The injury class codes are as follows:

NATURE OF INJURY

If the officer fills out the nature of the injury codes 2 through 7 the details of which part of the human body has the injury and type of injury should be present in the police report as well.

If a notice of infraction (citation) is issued the officer will enter the specific violation or Revised Code of Washington (RCW). Refer to http://apps.leg.wa.gov/rcw/ for a list of most updated State of Washington RCW’s.

  • The officer information will be located at the bottom of the Police Traffic Collision Report (PTCR) form. 
  • The officer is certifying under penalty of perjury, that the information recorded on the PTCR is true and correct and will have the following information:
  • Officer's signature. 
  • Unit or district.
  • Date and place. 
  • Approving authority signature and date
  • Badge number.
  • Time of officer dispatch and arrival at the accident scene.

  • 11. Probable vehicle and pedestrian paths, before and after the collision. Broken lines indicate the probable paths followed before the collision or point of impact. Solid lines will indicate paths followed after the collision or point of impact.
